Parents: if you received an email inviting your child to play on a 2008-2009 JBBA team, you can find the parent packet in the downloads section.

Please find the parent packet associated with the grade level that your son will play for, print it out and return the following forms at the first practice to Coach Boschee:

* Registration Form

* Medical Release Form (this form requires a notary)

* Liability Waiver (this form requires a notary)

* Payment Contract

*Sportsmanship Contract
